A blepharoplasty helps to remove the signs of aging around your eyes. Excess skin and fat are removed or redistributed. UV light, stress, smoking, genetics and natural aging processes lead to wrinkles, dark circles and hanging eyelids. 1) Before Your Eyelid Surgery

Plastic surgeon Dr. Sattler will define realistic aims with you and create an individual treatment plan. The main objective of a blepharoplasty is to remove excess skin and fatty tissues in order to create a more youthful look and a vital facial expression. To get the best aesthetic result possible, your blepharoplasty can be combined with other procedures, such as a liposuction, autologous fat transfer or an eyebrow lift.

An autologous fat transfer to the face in addition to a blepharoplasty adds volume to deficient areas and will rejuvenate your face. 2) Operating Techniques

A tightening procedure of the upper eyelids does not hurt very much and is usually performed on an outpatient basis under local anesthesia. If the upper and lower eyelids are operated together, general anesthesia is usually recommended. Depending on the size, extent and location of the surgery, you will decide together with Dr. Sattler whether local anesthesia or general anesthesia is the best option for you. Many different techniques to perform a blepharoplasty are known. Dr. Sattler will choose the best and safest method for you, depending on your anatomy and your aims and desires.

Usually, the fine scars can be hidden in your natural skin creases. It is important to avoid the “operated look” and to emphasize your natural beauty instead.

In an upper eyelid blepharoplasty, the incisions are placed directly in the transverse fold on the upper eyelid. This way buckling fat and excess skin can be removed nicely. Dr. Sattler uses an electric scalpel in order to operate very precisely and to minimize blood loss and swelling. After some weeks, the small scars are hardly visible. Your eyes will appear bigger and more alert after your surgery and you will look more youthful.

If your field of vision is restricted by your upper eyelids the upper eyelid blepharoplasty is not only performed for aesthetic reasons but also for medical reasons.

In the case of a blepharoplasty of your lower eyelid, a fine incision is usually made just below your eye lashes. Fat and excess skin can be removed under direct vision. In some cases, the fat is just redistributed and not removed. If there is no excess of skin, the incision can be made on the inside of your lower eyelid leaving no visible scars behind. In combination with a blepharoplasty, it often makes sense to perform an autologous fat transfer to regions with a volume deficit, for example to the temples, the cheekbones or lips. The extra volume will make you look more youthful and rested. In addition, wrinkles and scars can be injected with autologous fat and your skin can be revitalized by a stem cell transfer. 4) After Your Operation

At the end of the operation the wounds are dressed with fine tapes. Your eyelids might be a bit swollen and sometimes there are some bruises. Therefore, Dr. Sattler recommends to cool the operated areas, to elevate the head and to take some NSAIDs and enzymes if necessary. After about a week, the suture material can be removed. Sunbathing, solarium, sauna and sports should be avoided until all wounds are healed nicely. With make-up and sunglasses, you are usually able to socialize with your friends after a few days again.

5) Frequently Asked Questions About Blepharoplasties:

a) Do I need a general anesthesia for my blepharoplasty?

A blepharoplasty of the upper eyelids can almost always be performed under local anesthesia, even if it is a revision case. Usually the surgery lasts about an hour and is associated with very little blood loss. You will hardly need any pain killers. The procedure can usually be performed on an outpatient basis, but you should not drive yourself after the operation.

b) Can a blepharoplasty be repeated several times?

Yes, this is possible. Basically, upper and lower eyelids can be tightened several times. Depending on the clinical findings, more complex techniques have to be used in order to achieve a nice, safe, and long-lasting result, especially if there is increased scarring. Sometimes it makes sense to perform revision blepharoplasties under general anesthesia, especially if the lower eyelids are operated on.

c) Can my upper and lower eyelids be tightened in one operation?

Upper and lower eyelids can be operated together. However, in most cases a general anesthesia is recommended. Usually, a combined operation takes three to four hours. The duration of the operation varies, depending on the clinical findings and your goals. By combining two interventions, you will reduce your downtime. This can be very beneficial especially for mothers of young children.

d) Does it make sense to combine a blepharoplasty with other procedures?

If your eyebrows are placed too low, it is a good idea to combine a blepharoplasty with a brow lift. A forehead lift, midface lift, facelift or even a neck lift can also be combined with a blepharoplasty. To bring more youthfulness to your face, areas that lack volume, for example the temples, cheekbones and lips, can be augmented with autologous fat. In addition, a better contour of your face can be achieved by liposucking your chin and neck.
